Abstract:   Photo-thermal biomodulation of microfiltered adult mesenchymal stem cells from
     lipoaspirate: a new protocol for autologous exosomes release.

      Background: Clinical use of adult mesenchymal stem cells (MSCa) in medicine
      and regenerative surgery is constantly evolving. Adipose tissue-derived stem cells (ADSc) are
      capable of inducing the production of new extracellular matrix (ECM) deposition of new collagen
      and early revascularisation. However, poor engraftment, low survival rate of transplanted MSC,
      and impaired donor-MSC potency under host age/disease result in unsatisfactory therapeutic
      outcomes. Enhancement strategies, including genetic manipulation, priming, and modification of
      culture method, have been investigated to generate highly functional MSC.
      Photothermal biomodulation (PTBM) using simultaneously wavelengths from the red to near-
      infrared light spectrum (620 and 850 nm), blue light (467nm) and a specific temperature (37°C)
      induces mesenchymal stem cell activation and autologous exosomes release before the implant.
      Methods: Disaggregated and microfiltered adipose tissue was prepared with Milligraft kit
      (DualTrend, Italy). The ADSc are activated to release autologous exosomes with MCT® (Meta Cell
      Technology, Spain) that is a system specially designed to ensure the effective and safe delivery of
      photo thermal boost stimuli to biological tissues.
      Results: Even after filtration of the disaggregated adipose tissue (AT), significant number of ADSc
      was collected. Also in the non-filtered AT disaggregated highest number of cells was found, as
      expected, but at the expense of more pronounced inflammation. Harvesting of AT with a 16
      Gauge needle produces superior results compared to the cannula. The Authors demonstrate that
      this method of ADSc purification allows the subsequent photo-thermal biomodulation and
      autologous exosomes release with a standardized protocol and present 8 months clinical results.
      Conclusions: With this new method in medicine and regenerative surgery it will be easier to
      perform the isolation, purification and photo-thermal biomodulation of the ADSc as they are
      isolated from the inflammatory component.
